If you think you might be eligible for the NDIS, you can:

  • Make a verbal access inquiry by calling 1800 800 110, or
  • Fill out the ARF on your computer and email it to the NDIA along with supporting information, or
  • You can print the ARF and mail it to NDIA with supporting information, or you can
  • Send an email to NAT@ndis.gov.au and ask for an ARF.
  • Request an ARF from your local NDIA office.

Your compatibility with your service provider is crucial before selecting an NDIS provider. We at AEON Disability Services strive to meet all your needs in the most compassionate manner possible. However, below are a few factors to keep in mind prior to choosing your NDIS provider.

  • They must have excellent knowledge and skills in their respective NDIS domains.
  • They should have experience delivering similar kinds of services as you need.
  • They should have a participant-centric approach for service delivery.

NDIS participants are required to provide the following

  • Proof of identity (birth certificate or arrival document)
  • Your NDIS plan
  • Contact details of your emergency contact

Yes, absolutely. If you are dissatisfied with the services provided by your NDIS provider, you can end your contract with the provider by using the MyPlace portal for NDIS participants online. After this, you can choose a new provider as per your liking.
