If you think you might be eligible for the NDIS, you can:
- Make a verbal access inquiry by calling 1800 800 110, or
- Fill out the ARF on your computer and email it to the NDIA along with supporting information, or
- You can print the ARF and mail it to NDIA with supporting information, or you can
- Send an email to NAT@ndis.gov.au and ask for an ARF.

Yes, absolutely. If you are dissatisfied with the services provided by your NDIS provider, you can end your contract with the provider by using the MyPlace portal for NDIS participants online. After this, you can choose a new provider as per your liking.
